“I say: Fear not! Life still Leaves human effort scope. But, since life teems with ill, Nurse no extravagant hope: Because thou must not dream, thou need'st not then despair”-Matthew Arnold, Empedocles on Etna.
This is how William Osler began his address to the new students about to embark on the journey to becoming physicians. He emphasized that they would need to develop two qualities: imperturbability meaning coolness and presence of mind at times of crisis and equanimity meaning the absence of emotions. This is an essay which is worth reading by all students of medicine.
Other articles in this issue take up the latest in stethoscopes with electronic augmentation, ICD guidelines are reviewed, and catheter-based interventions for pulmonary embolism are reviewed from a tertiary center. Prosthetic valve thrombosis is a major problem, and the therapy is highlighted with a series on thrombolysis and also a series which attempts to tackle the problem by improving International Normalized Ratio (INR) by education. In other parts of the journal, lung transplant, a new approach to therapy with antisense oligonucleotides, discharge counseling for heart failure, and the history of genetics are discussed.
We continue to remind our readers that this is a journal for teaching, so whenever you have a point you wish to make, a case series you wish to present, a scribble you wish to send to others, send it across and ask your students to do the same.
